Cathay Life Insurance claims apply requirements document

• 醫生開立之診斷證明書正本
• Medical certificate (Original)

• 收據正本或副本。(若為影本需加蓋院章)
• Original receipt (If photocopy there must be hospital verified stamp)

• 護照或居留證影本
• Passport or ARC photocopy

• 若有骨折，請附 X 光片/碟
• ​If there’s bone fragment, please provide X-Ray/CD
